Pagetoid cells in pagetoid patternを示す腫瘍 ～Paget病、上皮内悪性黒色腫、Bowen病の鑑別
表皮内や粘膜上皮内に胞体が淡染する異型性のある腫瘍細胞が胞巣形成や散在して存在している場合(Pagetoid cells in pagetoid pattern)、Extramammary and mammary Paget’s diseaseのほかに、Malignant melanoma in situ(上皮内悪性黒色腫)、Bowen病の可能性を考える必要がある。多くの場合はHE染色標本で鑑別可能だが、迷ったときには免疫組織化学染色が威力を発揮する。Paget’s diseaseは、CK7、CAM5.2が陽性で、Malignant melanoma in situはS-100蛋白陽性、Bowen病はいずれも陰性である。 染色がきれいなので、我々はCK7を好んで使っている。GCDFPやCEAはExtramammary Paget’s diseaseでは多くの例で陽性だが、Mammary Paget’s diseaseでは陰性例も多い。CK7を使っていて問題が生じたことはないが、CK7陽性のBowen病も報告されているので注意すべきだろう。
