a) 未分化大細胞型リンパ腫 anaplastic large cell lymphoma
60-85%に陽性で、cytoplasmic とnuclearに染まるパターンがある。t(2;5)/NPM-ALK translocationを示す症例のほとんどはcytoplasmic and nuclear の両方とも陽性となる。この陽性像はtranslocationのパターンによってはcytoplasmicのみ、あるいはnulearのみと変化する。(WHO Tumors of Haematopoietic and lymphoid tissues, 2001, p230-235)。ALK陽性ALCLは若年者に多く、治療によく反応し、５年生存率は90%(Shiota M et. al. Anaplastic large cell lymphomas expressing the novel chimeric protein p80NPM/ALK: a distinct clinicopathologic entity. Blood. 1995 Sep 1;86(5):1954-60, Nakamura S et.al Anaplastic large cell lymphoma: a distinct molecular pathologic entity: a reappraisal with special reference to p80(NPM/ALK) expression. Am J Surg Pathol. 1997 Dec;21(12):1420-32)。陰性例はheteroな集団と考えられる(森茂郎、中村栄男 未分化大細胞方リンパ腫 in菊池 昌弘、森茂郎 監修 最新・悪性リンパ腫アトラス 文光堂 p266-275)。ALCLでは必ずALKを染色すべきである。なお、cutaneous formのALCLは陰性である。
b) 炎症性筋線維芽細胞性腫腫 inflammatory myofibroblastic tumor:IMT
(Griffin CA et.al. Recurrent involvement of 2p23 in inflammatory myofibroblastic tumors.Cancer Res. 1999 Jun 15;59(12):2776-80)に陽性が知られる。
その他の腫瘍にも陽性が知られ、例えばLi XQらの249例のsarcomaにおける検討においてはIMTs (4 of 4), rhabdomyosarcomas (4 of 7), various lipogenic tumors (35 of 65), Ewing’s sarcoma/peripheral primitive neuroectodermal tumors (6 of 10), malignant fibrous histiocytomas (8 of 37), leiomyosarcomas (3 of 18), and other non-IMT tumors (9 of 108)と、IMTよりも弱いながら、様々な腫瘍に陽性像がみられたという(Li XQ, Hisaoka M, Shi DR, Zhu XZ, Hashimoto H. ※肺癌のEML4-ALKの検出には適さない。ALK (D5F3および5A4：非小細胞肺癌コンパニオン診断薬)を参照のこと
