原発性外陰部Paget病と２次性Paget病の鑑別

CK7とCK20の組合せは、肛囲や、尿道口周囲のExtramammary Paget’s disease(乳房外Paget病)と、直腸癌や肛門管癌、尿路上皮癌が肛囲や尿道周囲の皮膚や粘膜内に進展したSecondary extramammary Paget’s disease(２次性乳房外Paget病、Pagetoid spread of internal malignacy)を鑑別するのに有用である¹。

Extramammary Paget’s diseaseの腫瘍細胞は、CK7陽性、CK20陰性なので、CK20陽性の腫瘍細胞が確認できれば、Secondary extramammary Paget’s diseaseの可能性が高いと判断できる。

GoldblumとHartは、CK7+, CK20+のExtramammary Paget’s diseaseもまれにあり(2/19)、CK7は２次性Paget病でも陽性になるので、GCDFP15も加えて検討するのがよいと述べている^2、3。GCDFP15は多くのExtramammary Paget’s diseaseで陽性となるが、Secondary extramammary Paget’s diseaseでは陰性である。
