推奨陽性コントロール:卵巣漿液性腺癌、その他下記に示す正常組織。

 

trascription factorのひとつで、 甲状腺, 腎臓, Müllerian systemのembryonic developmentおよび、これらの臓器の腫瘍形成に関与する

正常組織では下記が陽性(Mod Pathol. 2011 Jun;24(6):751-64. Epub 2011 Feb 11.PAX 8 expression in non-neoplastic tissues, primary tumors, and metastatic tumors: a comprehensive immunohistochemical study. Ozcan A, Shen SS, Hamilton C, Anjana K, Coffey D, Krishnan B, Truong LD.

糸球体上皮細胞、萎縮性尿細管、頚管腺上皮、内膜上皮、卵管上皮、精嚢上皮、精巣上体上皮、甲状腺上皮、膵島細胞、リンパ球

 

1)Mullerian carcinomaに特異度高く陽性(Diagn Cytopathol. 2010 Oct 14.Diagnostic utility of PAX8 and PAX2 immunohistochemistry in the identification of metastatic Mullerian carcinoma in effusions.Wiseman W, Michael CW, Roh MH.)(PAX8: A marker for carcinoma of Mullerian origin in serous effusions.Tong GX, Devaraj K, Hamele-Bena D, Yu WM, Turk A, Chen X, Wright JD, Greenebaum E.)。] これまで免疫組織学的には鑑別困難であった卵巣漿液性腺癌と悪性中皮腫を鑑別可能とする(Am J Surg Pathol. 2010 May;34(5):627-35)

Lauryらの検討によれば以下の通り

high-grade serous ovarian carcinomas 99%
low-grade ovarian carcinomas and serous borderline tumors 100%
pleural malignant mesotheliomas 0%
peritoneal malignant mesotheliomas 9%(focal or weak)

男性の健常組織においては、曲精細管, 精巣門, 精巣上体, 精嚢, 射精管, 中腎管遺残などに陽性。腫瘍ではミュラー管、ウォルフ管由来どちらでも陽性と思われ、例数の少ない検討ではあるが精巣上体のserous cysradennoma, 精巣adenocarcinoma, 精嚢adnexal tumor of probable Wolffian origin, 精嚢endometrioid adenocarcinomaに陽性。(PAX8 and PAX2 immunostaining facilitates the diagnosis of primary epithelial neoplasms of the male genital tract. Tong GX et.al Am J Surg Pathol. 2011 Oct;35(10):1473-83.)

2)膵臓ではwell differentiated endocrine tumorsの3/4に陽性だが、solid pseudopapillary neoplasmの1/4にも陽性であることに注意(Mod Pathol. 2010 Oct 1. PAX8 expression reliably distinguishes pancreatic well-differentiated neuroendocrine tumors from ileal and pulmonary well-differentiated neuroendocrine tumors and pancreatic acinar cell carcinoma.Sangoi AR, Ohgami RS, Pai RK, Beck AH, McKenney JK, Pai RK..)

3)腎細胞癌(Mod Pathol. 2009 Sep;22(9):1218-27. Epub 2009 Jun 12.Expression of PAX8 in normal and neoplastic renal tissues: an immunohistochemical study.Tong GX, Yu WM, Beaubier NT, Weeden EM, Hamele-Bena D, Mansukhani MM, O’Toole KM.)。また、PAX8(+)/p63(-) immnostaining patternがcollecting duct carcinomgの特徴であり、尿路上皮癌との鑑別に有用(Am J Surg Pathol. 2010 Jul;34(7):965-9.)
